“There is a wisdom of the head, and… there is a wisdom of the heart.” — Charles Dickens

A Plan in Place

A little support. A clearer way forward.

When you or someone you love needs more help, it can be difficult to know where to begin.

A Plan in Place offers older adults and their families a place to start: someone to listen, explore what is possible, and gather the next steps into a practical written plan.

Let's Start with a Conversation

How A Plan in Place Works

We start with what matters to you.

I meet with you or your loved one in person to hear what is working well, what feels difficult, and where a little help could make everyday life easier. Together, we identify up to three priorities to explore.

Your wishes, routines and preferences guide the conversation. Family members can be involved with your permission, while keeping the older adult at the centre of the plan.

 We explore what is possible.

I research services and resources related to your priorities, considering your location, preferences and budget. That might mean meal services, transportation, non-medical help at home, or opportunities to stay connected with others.

You will see what could be helpful for your situation, along with available pricing, so you can make informed choices about the help you would like.

Steps

We map out your next steps.

I gather the research into a written plan with relevant resources and suggested next steps, usually within two weeks of our visit. Then we go through it together, talk about your questions, and identify where you would like to begin.

You do not have to do everything at once. The plan gives you a starting point and something to refer back to as you move forward.

Support that grows through knowing you.

Once your plan is in place, you can choose to continue with a monthly phone call or a monthly personal visit. Each time, we talk about what is working, what is new, and adjust next steps as your needs or wishes change.

With your permission, a family member can be kept informed. Continuing month to month is always optional.

Let's find your place to begin

You do not need to have everything figured out before reaching out. Start with a complimentary 30-minute introductory call to share what is on your mind and see whether A Plan in Place is a good fit. If it is, I will explain the fees for your plan and any monthly follow-up before we go ahead.
